How It Works

Scope

A short intake call with Dr. Chance to confirm who is being assessed and what decisions the results need to inform.

Assessment

Each participant completes the guided questionnaire — roughly 25 minutes, answered independently.

Scoring

Responses are scored across the five areas, with gaps between leadership and staff answers flagged.

Results Summary

A written report: scores by area, strengths, risks, and the findings worth acting on first.

Debrief

A facilitated session to walk the results, resolve disagreement, and agree on a 90-day focus.
